APPOINTMENT INFO
- Please make your appointment by entering this site and going to Schedule Appointment OR email Beth at Beth@AvDoc51.com OR call 407-846-0854.
- Prior to your scheduled appointment for an FAA Airman Medical Certificate Evaluation, Class 1, 2 or 3--please complete the Application for an FAA Airman Medial Certificate by logging into www.MedXpress.FAA.Gov where you will be filling out the FAA Form 8500-8. You will be asked to create a new account and password, unless you have already done so for previous certificates.
Completing the Application for Airman Medical Certificate, FAA Form 8500-8 (all pilots read-please)
- PLEASE READ IN FULL SO THAT YOU DON'T ATTRACT NEGATIVE ATTENTION FROM OUR FAA FRIENDS
- Block 3--Provide only your full legal name in Block 3 to include middle name as it appears on your birth certificate, passport, drivers license or pilot license--no nicknames no abbreviations no initials
- Block 17b, please read this very carefully, do you wear "CONTACT LENSES FOR READING WHILE FLYING"--that is contact lenses for READING/NEAR vision (not distance vision) while flying; it is very unlikely that someone would wear contact lenses for near-reading vision while flying, so the answer would be NO
+++Block 18, "HAVE YOU EVER IN YOUR LIFE", that's right, in your life since your were born--this medical history in Block 18 does not expire, ALSO, putting PRNC or previously reported no change is NO LONGER HAPPILY ACCEPTED BY FAA so please briefly-briefly explain your yes answer in the comment block.
- Block 18y, Disability Benefits--this includes any disability benefits provided to you from Social Security, Veterans Affairs--military VA rating or any other state, county or business oriented disability benefits. IF those benefits were temporary and eventually terminated then put that in as a yes and explain in the comments block, Be truthful and complete since the FAA will compare records with the VA and Social Security and approach you if the info does not coincide.
- Block 19, health care provider (doctor) visits within the past THREE YEARS (3 years), use your judgement or discuss with our staff, for instance if you were seen several times for the same affliction then list it once and explain the multiple visits in the comments section, we can help you with this.
- Block 20, this authorizes the FAA to check on your driving record with the National Driver Registry (NDR) and with your state Department of Motor Vehicles (DMV)--that is at the state and national level of your driving record. Please note Block 18v, read this carefully and acknowledge if it pertains to any of your driving history, the FAA is mostly interested in your history of operating motor vehicles of any type, any type under the influence of a substance (drugs/alcohol), please be truthful since they will find out anyway.
- Once fully completed, then Submit the Application in accordance with the website directions and copy down your confirmation number (usually +/-12 numbers).
- ALSO--and this is important--Please print out a hard copy of your completed Med Express Form 8500-8 and bring that to your appointment, we must see this before your exam can be completed.
- If you have any problems or uncertainties or questions about your application, then complete it as much as you can and then save it according to instructions, then when you come in for your appointment we will pull it up and look it over before submission.
- ****VERY IMPORTANT NOTE: If you are not computer savvy and have problems with negotiating web based forms or information or you do not have the computer equipment or computer skills at your disposal or for some reason you can not access the FAA MedXpress site then please call and we will make arrangements for you to complete your MedXpress Form 8500-8 here at our office using our computer and under the guidance of our staff, service fee may be applied.
- Please bring the following to your scheduled appointment:
- Identification-passport or drivers license and pilot license-foreign equivalents are okay.
- A list of all medications and dosages to include OTC-over the counter.
- Prescription glasses and contact lens case with solution, if you have your glasses or contact prescriptions written down that would also be vey good.
- A printed out paper copy of your completed FAA Form 8500-8 (as previously noted), if a printer is not accessible bring your confirmation number and we will help you here.
- Please be on time-not too early and not late--on time.
Before your appointment:
- Do NOT use caffeinated products like energy drinks, Starbucks, coffee, tea, soda, decongestants, or other stimulants for several hours.
- Do NOT use moisturizing lotions or other types of products on your body if an EKG is planned--the EKG tabs won't stick.
- Do NOT exercise/run a lot or use sauna/steam room for several hours before your appointment. Make sure that you are well rested and well hydrated.
- Bring some pictures of the aircraft you fly or plan to fly in print or on your phone, we really enjoy seeing those as we try to have some good conversation and fun during our work day.
- If your family members, children or friends are with you they do not need to wait in the car, we have plenty of room for them to be inside our air conditioned office spaces, if you bring an animal please make sure it is well behaved and on a leash.

Unfortunately we must charge a service fee if you do not show up for your scheduled appointment
Here is Why---REASONS FOR NO SHOW SERVICE FEE
We do not want to charge a no show fee, but the people at the top do and here are some reasons. If you schedule a time for your appointment, then you have covered that time spot and that time is not available to any other pilot and is held for you only. If you do not show for your appointment then you are preventing another pilot from using that time slot. Also, the clinic staff is expecting that since you scheduled at that time then you would show up at that time. If you fail to show up then the staff is waiting for you and wondering why you are not there at your scheduled time. In addition, the overhead costs of maintaining the business; the fees for running the office (building maintenance, office space rent, electric, water, perishables, equipment maintenance, insurance, taxes, payroll, lawn care, pest control, housekeeping, pencils, pens, toilet paper, urine cubs, etc etc) adds up and still keeps accumulating even though you did not show up for your scheduled time and the staff is waiting for you and you prevented any other pilot from having that appointment time. So please, if you schedule yourself and cover a time slot, please plan and do your best to make your appointment. Come prepared, (see what to bring above) and be on time. We will all have a good experience and do our best at getting your FAA Medical Certificate and in making you happy. NOTE: if extenuating circumstances preclude you from making your appointment, please call Beth as soon as you can and explain. She is very reasonable and perhaps we can put someone else in your time slot. See you soon.
